Abstract

The data collections of child development in health centers are less efficient and effective. All the immunization and weight data of the child are done manually. If data collection is still done manually, the information will be less accurate because of   many possible errors in the data collection process. The other problems of the manual data collection system are a data input error, the length of time report, and disorganized documentation report. Based on those problems, it is necessary to design a Computer-Based Growth Data Collection System that can process data and report a child development. The data collection methods are observation, interview, library, and documentation research. Observation and interview are conducted directly on the objects of the research, whereas the library and documentation research are taken from the research of books, documentations, and the literature related to the issue. The methods or stages in system design are planning system, analysis system, design application system, implementation system, and testing. The result of the analysis and design is Child Growth Data Collection System Design.  Key words: Immunization Data Collection, Weight Loss Data Collection
